All subcontractors and suppliers are required to complete two forms. 1. Company Information Form and 2. Subcontractor/Supplier Qualification Application. The application will be used by Clark to aid in determining the qualifications of a subcontractor to perform the specific work for which it is being considered. Clark will not provide such information to third parties, except as required to secure Subcontractor's performance in accordance with its Subcontract(s).
